I had to support Dragon at the last place I worked. New special needs students (ones who couldn't type or write very quickly) were given a half day to just train dragon for use. This was done in the afternoon and supervised by a special education teacher. As A.J. said it can take hours to properly train Dragon to understand how the user speaks. We used some really cheap headsets with it.
As for a desk microphone... blue snowball all the way.

We love our snowball here. It has literally gone all over the world with us and been used for Skype chats to recording music demos.

I don't know of a Bluetooth microphone that would be reliable. Unless you did it via a cell phone headset such as the Voyager that we have mentioned

This is so close, yet so far from what I want. While it's wireless, the handheld will only pair with it's own basestation, so it kills it for our use.

I missed this the first time I read your post. You'd be surprised. My docs have probably spent less than 1 hour each training Dragon. We've been using it for the past 7 years. With the medical edition, they sat down, read about 5 pages of text and that was about it. They didn't want to be trained in any extra functions. They either dictate into the dictation box or in most cases these days they are able to dictate directly into the field where the information needs to go.

That is VERY surprising. How much time must be spent editing the dictations?

Surprisingly little. Granted it's not perfect, but they don't want to buckle down and LEARN the system either..

Wow, it must have gotten much better then when I was supporting it. Good to know.

Over all I'm pretty happy with the way it works. The medical edition does a pretty darn good job with the medical words to without any additional training.
Though one doc couldn't get the system to accept vasectomy. We had to train that one for about 3 mins, but since it's been good.
The other key is to speak punctuation. Don't bother trying to have Dragon figure it out.
